Information for New Faculty & Staff
Access new employee forms. Please visit the Faculty & Staff page for information on benefits enrollment, payroll, timesheets and other procedures.
- NetID & Password: Shortly after you accept the job offer, the email address you provided upon application will receive instructions for activating your UAlbany NetID and password, which will give you access to your University email inbox and other important logins. Please activate your account as soon as possible. Learn more about managing your account.
- Orientation: You’ll also receive an email (sent to both your UAlbany email inbox and personal email inbox) with information on new employee orientation, including where and when your session will be held. Orientation is typically held in person and covers new hire forms, benefits and more.

Information for New Student Employees
Access new employee forms. Please visit the Student Employees page for information on payroll, timesheets, benefits and other procedures.
Additionally, new graduate assistants and teaching assistants will be invited to attend a one-hour orientation webinar.
Required New Employee Forms
Note: Employees rehired after a break in service of one year or more are considered new employees and are required to complete new employee forms.
Please submit all forms via mail or in person (University Administration Building, Suite 300) on or before your start date. Do not email or fax files that contain sensitive information, such as Social Security numbers.

Required Forms for All New Employees
All new employees — including professional, classified and student employees — are required to complete the following forms:
- New Employee Information Form (EMP-2)
- Employment Eligibility Verification (Form I-9)
- All employees must complete Section 1 of Form I-9 to verify their identity and authorization to work in the United State. This form must be completed no later than the first day of employment.
- Review the list of Acceptable Documents, to present original documents. Screenshots and copies will not be accepted.
- Section 2 of I-9 must be completed by an authorized representative. DO NOT email or mail the form if Section 2 is incomplete.
- How to complete Section 2:
- In person at the HR office or with supervisor OR,
- Through an authorized institution for employees located outside of the area. To coordinate email [email protected]
- Oath Card
- Payroll Withholding Forms (W-4 and IT-2104)
- Direct Deposit Enrollment Form (AC 2772)
All new employees must also complete any additional appointment forms provided by their supervisor.
Additional Required Forms for New Graduate & Teaching Assistants
New graduate assistants and teaching assistants are also required to complete the Employees' Retirement System (ERS) Info Sheet and Acknowledgement.
Additional Information for New International Employees
International employees must schedule a time to complete all of the required forms with Valerie Goris via Calendly.

Required New Volunteer Form
All new volunteers must submit a completed New Volunteer & Unpaid Employee Information Form (VOL-2) via mail or in person (University Administration Building, Suite 300) on or before their start date. Do not email or fax files that contain sensitive information.
Foreign nationals visiting the U.S. on a visa must be in a work-authorized status to volunteer at UAlbany. Visa holders must attach proper visa documentation to their completed form and submit both before they start volunteering.
Forms without proper documentation will not be processed.
