
Adirondack Rural Health Network Logic Model Development and Technical Assistance Project
This is the second year CHSR is working with the Adirondack Rural Health Network (ARHN) to assist mini-grant recipients in developing logic models and evaluation plans. Mini-grant funding is intended to further the objective of the ARHN’s Community Health Planning Committee’s selected priority area for 2010-2014 and to bring the region closer to the New York State Department of Health’s Prevention Agenda goals for physical activity and nutrition.
Program Development
CHSR provided a hands-on workshop on logic model development for mini-grant applicants. Funded projects were then offered individualized technical assistance on their evaluation plans. CHSR is also providing consultation and technical assistance to the Hospital Subcommittee as they develop an online community-based survey of the Adirondack regional community.
