Lactation Care Management in Urban and Rural Primary Care Settings: Ensuring Continuity of Care for Breastfeeding and Chestfeeding Families

Overview

Gaps exist in the continuity of care for breastfeeding and chestfeeding families, particularly in primary care settings. This webinar will review these gaps and their root causes and explore the role of a community lactation coordinator to assist pregnant and postpartum families in navigating and accessing, in a timely manner, appropriate community services that primarily serve families experiencing the greatest breastfeeding and chestfeeding inequities in the community.  Two health care practices will share information from their experiences implementing lactation care management programs in rural and urban settings, and how the programs can be an effective approach to reduce breastfeeding and chestfeeding disparities. Participants will have an opportunity to discuss how communities can adopt and implement a lactation care coordinator role to support families with their breastfeeding and chestfeeding goals.

As a result of participation in this activity, the learner will have increased and enhanced their knowledge and competence on strategies for improving continuity of care and disparities in breastfeeding/chestfeeding in primary care settings.

By the end of the activity, learners will be able to: 

  • Summarize two different data driven models of lactation care management within NY State.
  • Describe how lactation care management works within a larger health care system and community.
  • Identify at least two barriers to breastfeeding/chestfeeding initiation and duration that can be addressed by lactation care management.  
  • Discuss how this type of program can be integrated into primary care settings.

Each participant interested in CE credits must watch the activity, complete an evaluation and post-test, which is available above under "Evaluation, Post-test for CE Credits". A score of 80% and above on the post-test will generate a certificate indicating the requested credits.
