Monitoring and Evaluating Evidence-Based Self-Management Programs

Increasing the availability, accessibility and use of evidence-based interventions to prevent or manage chronic disease is a key strategy within the Prevent Chronic Diseases focus area of the New York State Prevention Agenda. The value of these programs and progress toward achieving Prevention Agenda outcomes can be demonstrated through ongoing performance measurement and program evaluation. This webinar will introduce the concepts of evaluation and performance measurement as they relate to evidence-based self-management programs for chronic disease. Participants will learn how to define activities, develop evaluation questions, collect data and monitor progress around the delivery, promotion of and referral to evidence-based self-management programs;  a local organization’s use of data to monitor and evaluate a Diabetes Prevention Program intervention in community and health care based settings will be highlighted.

Learning Objectives
After this program participants will be able to

  • Describe evidence-based self-management programs for the prevention and management of chronic disease are and be able to list examples;
  • Define program evaluation and performance monitoring as it relates to evidence-based interventions and explain why they are important;
  • Identify existing resources to evaluate and monitor three aspects of implementing evidence-based self-management programs; and
  • Review an example of a local evaluation.
