Logic Models
A logic model is a systematic and visual representation of how a program works by linking outcomes (both short and long term) with program activities and processes. It incorporates the resources available to operate a program, the sequence of activities involved in service delivery, and it shows how these activities are linked to the results the program is expected to achieve. Increasingly, funders are requiring the incorporation of logic models in proposals. CHSR works with service providers to develop logic models that specify the program model and also help to describe the evaluation plan and the types of data that will be collected. Performance Targets, Indicators, and Outcome Measures
Funders often seek clear descriptions of how programs demonstrate their effectiveness. Providers must report more than simply the numbers of participants reached or sessions held. CHSR assists organizations in: developing performance targets that define progress towards goals, identifying reliable and valid indicators of a program’s efficacy, and specifying outcome measures to report on what a program has achieved.
Proposals and Grant Applications
CHSR prepares proposals and assists organizations to locate grant opportunities that enhance services and that include evaluation components. CHSR partners with organizations to prepare and submit grant applications to foundations and to local, state, and federal government agencies.
