This page will guide you through the Human Resources forms required for the appointment of Graduate &Teaching Assistants and Fellowships. Additional information for students and faculty can be found on the Graduate School web page.
Full Time (20 hours per week): $10,158

HRM-4 - Appointment Form - required for new hires, rehires, changes in status, and terminations
EMP-2 - New Employee Information Form - not required for rehires/renewals
Internationals Students should email [email protected] to request certification of their Form I-9 before they commence work duties.
Form I-9 - Employment Eligibility Verification - this link takes you to a U.S. government site, which has the most up-to-date version of the form and download instructions. This form is required for all new employees or employees rehired after a break in service of a year or more
Employees must present original document(s) (listed in the instructions) on or before their first day of work. This form should be held at the department until original documents are presented (remaining appointment forms should be forwarded to HR). International students should bring this form to HR on or before their first day of work.
Tax Forms - these forms are required for all new employees or employees who need to change their tax withholding status
Form W-4 - Federal Tax Withholding or NRA Form W-4 for international students
Form IT-2104 - State Tax Withholding or NRA Form IT-2104 for international students
