If you have made a correction to your FAFSA, and the Financial Aid Office has determined you have unsubsidized Federal Stafford Loan eligibility, you may use this form to request a review of your eligibility for a subsidized Stafford loan.

The Financial Aid Office may request additional documentation before being able to make a determination of Subsidized Stafford eligibility. Please monitor your MyUAlbany To-Do List for any documentation you may need to provide.
