Documentation Requirements
Students selected for QAP will be asked to submit documentation to verify what was reported on the student's FAFSA.
In June, QAP students are notified of their QAP selection through postal mail, with follow-up email reminders. QAP students have 30 days to comply with the documentation request.
Required documentation for federally dependent students selected for QAP:
a signed copy of the student's federal tax return for the specified year
Student W-2 Forms for the specified year
a signed copy of the parent's federal tax return for the specified year
Parent W-2 Forms for the specified year

Required documentation for federally independent students selected for QAP:
a signed copy of the student and spouse (if applicable) federal tax return for the specified year
Student W-2 Forms for the specified year
a signed copy of the parent's federal tax return for the specified year

QAP required documentation must be returned by the deadline date specified in the request.
The deadline to submit QAP documents for the 2008-2009 aid year is July 7, 2008.
