Healthy Choices New York
Healthy Choices NY is a statewide initiative to empower older adults by providing evidence-based health promotion programs in community-based settings. Since 2007, Healthy Choices NY has served hundreds of New Yorkers by implementing evidence-based programs such as “Living Healthy” (the Chronic Disease Self Management Program), “A Matter of Balance” and “Active Choices,” which use workshops and personal coaching to encourage healthy living. Participating area agencies on aging, Retired Senior Volunteer Programs and Naturally Occurring Retirement Communities have been very successful in extending the reach of evidence-based health programming through partnerships with local departments of health, community service providers, housing providers, senior centers, health networks, insurers, and faith-based communities. This initiative is funded through the U.S. Administration on Aging, the NYS Office for the Aging, and the NYS Department of Health.
