• CoRe: Community Reengagement Program - This is a program designed to reengage students who are currently on Disciplinary Probation with the University community.  By completing this program students on Disciplinary Probation will have the opportunity to end their probation.  Below is all the documentation needed to complete the program.  Please read all the documentation carefully.

• Release of Information Form – allows staff to discuss your conduct case with the individual(s) named on the form.
  
 
• VAIR – Violation of Academic Integrity Report – used by faculty to violations of the academic integrity codes to the Undergraduate and Graduation Education Office.

• Alcohol Policy & Alcohol Use Request Form - explains the University at Albany's Alcohol Policy and contains the Alcohol Use Request Form. The request form must be completed and approved for any student, faculty or staff member to host an event on campus where alcohol will be present.  The request form must be submitted to one's respective Local Alcohol Policy Administrator and a copy of the request must be faxed to John Murphy, Associate Vice President for Student Success.
