Forms can be turned in at CC137 in the Campus Center:
- Between 8:30 AM and 5:00 PM during the semester
- Between 8:00 AM and 5:30 PM during finals

Scholarship Application: | PDF | MS WORD |
Students may complete the form as a Word document and send it to the office as an Word document attachment via email OR they may scan the printed completed form (Accommodation Request, Testing Schedule, Scholarship Application) and email it to us as a PDF attachment to aarminio@uamail.albany.edu.
