By completing and submitting this form you are requesting that the ITS Web Office set up a process where data from a HTML form is placed in a file.

A manual approval process is required to ensure you are authorized to request this service. Currently, you must be a faculty/staff member of the University, a graduate student doing dissertation research, or your request must be for an approved student group. If requesting this service for a student group, the HTML form must reside on the student group's Student Organization Web Account (SOWA). Contact the Office of Student Involvement and Leadership for information concerning SOWAs

Notification for approval or rejection will be sent to the requester's email address.

Location of data file: Note:  Your data file must reside in a subdirectory within the home directory of your ITS UNIX account.

Data file name: Note:  This is the name of the file where the data from the HTML form is written. It will reside in the subdirectory you named above. An example data file name:  "surveydata.txt"

The default maximum size of your data file is 2000 kilobytes. Use 2000 kbyte default, or Specify custom kbyte value (higher or lower than default).
