INSTRUCTIONS:
To request assistance from Information Technology Services, please submit this form to the ITS Service Desk, our automated request system. Your submission will auto-route to the appropriate ITS team based on the information you provide and the concern you select from the drop-down menu. The Service Desk system will send an auto-reply message to confirm registration of your request with ITS. Should you need to include a file attachment (limit 900K) with your request, wait for the auto-reply message containing the incident number of your new request and then reply to add the information.
