All forms can be viewed, printed and filled out by hand. It is also possible to type within some fields in the form online. However, they must be submitted. There is NO save mechanism. After typing, you must print the form. If the browser closes, you will lose any data that was input. OEM will accept completed forms with ORIGINAL SIGNATURES only. PDF files require the free Adobe Acrobat Reader. If you are unable to view any of these forms online, please contact the Office of Equipment Management for a hard copy.
