  Request a mid-term survey of my students to get their feedback

Instructors who want to find out how students are responding to an ongoing course, with the idea of making mid-semester adjustments, may want to request a “Midterm Survey” of students. All survey data are confidential.

The process: early in the semester choose from one of the 7 versions of the survey (see below) that are currently available. Then fill out the form below with the information requested. The forms will be printed and available for you to pick them up the day before the scheduled survey. With advance planning, we may be able to deliver the forms to your office.

We recommend that you distribute the survey at the beginning of class and then wait outside the room while students respond. Pens or pencils work equally well. If you are concerned about student sensitivity and confidentiality, then simply ask a student to collect the forms and deliver them to ITLAL on your behalf. The data will be processed into an electronic report (click here to see sample) and made available to you within one week. ITLAL consultants are available to review the results with you, if you choose.

Instructors are asked to arrange for surveys one week in advance of the targeted survey date. Mid-term Surveys will not be available after the mid-point of the semester, in order to avoid the perception of conflict with or influence upon the official end-of-semester SIRFs.
