Prevention Agenda 2013-2018:
NYS Health Improvement Plan
The Prevention Agenda is the blueprint for state and local action to improve the health of New Yorkers in five priority areas and to reduce health disparities for racial, ethnic, disability, socioeconomic and other groups who experience them.
The target audience for these materials and resources are NYS Local Health Departments, Hospitals and Community-Based Organizations who are working together to
develop and implement their Community Health Improvement Plans and Community Service Plans.
Note: The Prevention Agenda 2013-2017 has been extended to 2018 to align its timeline with other state and federal health care reform initiatives.
