2009-2010 Waiver Application Deadlines
The deadline for medical insurance waiver applications is Sept. 25, 2009. All applications must be received by the Office of International Education no later than Sept. 25, 2009.
Waiver applications must be submitted in the Fall of every year for the current Academic Year, even if you have been waived out of the mandatory Medical Health Insurance in the past.
Waiver applications can be submitted for the Spring/Summer 2010 term beginning December 2009 and must be received by Friday, February 26, 2010.

You can drop off the documents to the Office of International Education in the Science Library (SL-G40), FAX them to 518-591-8171 Attention: Ms. Mason, or mail them to the address on the right.
Please make sure all documentation (front and back copy of insurance card and copy of insurance coverage) is attached and the application is completely filled out and signed.
Incomplete applications will not be viewed.
NOTE: If you have Life Insurance, Travel Insurance or Property Insurance, please do not complete a waiver application request. Those types of insurance do not meet the SUNY requirements and will not be accepted as a substitute for the International Medical Health Insurance.
