is expected to award a degree to you.
If you will have
received more than one degree, please
photocopy this form.
Because your
Pre-Health evaluation cannot be prepared
until the report of the Office of Judicial
Affairs is received, please read and sign
the top half of the Office of the Judicial
Affairs report form below. Include this
signed form in your Pre-Health file which,
is due the first Monday of February.
