All forms are to be submitted to the Pre-Health Advisor in LI 36. Refer to Pre Health Professions Time-Line for more information regarding the Application Process.

Submit to open your pre-health file:

Submit Junior Year by the First Monday in February:

• Grade Point Average Calculation Sheet (PDF format), web-based excel format
• Request for a Committee Evaluation and Interview
• Authorization and Waiver Form
• Schedule Card
• Evaluation Form
• Judicial Clearance Form

Submit during the Summer after Junior Year:

• Active Record Sheet
• Pre-Health Virtual Evaluation Fee Transmittal Form
