How To Apply

Please review the program's prerequisites to confirm your eligibility. The program will be accepting applications for the following year starting September 1 of the previous year. The applications will be reviewed on rolling basis starting in January. Selected candidates will be interviewed and notified of acceptance in February-March. Open positions may be filled later in the year.

All applications should be submitted on paper or via email ( directly to the program. All transcripts and letters of recommendation must be received directly from the source.

Your complete application should include:

Personal Statement (2 pages max.) describing your interest and past experience in public health and your career goals.

Curriculum Vitae

At least 3 Letters of Recommendation; one of these letters must be from your program director if you are a recent resident or fellow. If you are currently employed, one letter should be from your supervisor.  All letters should be up-to-date and sent directly from the source, not through the applicant.

Previous Residency Diploma (photocopy or scan)

Medical School Transcript

Graduate School Transcript (if applicable)

College Transcript

Note: All transcripts must come directly from the school, not through the applicant.

Copy of current NYS medical license (if licensed)

Proof of board certification or board eligibility in your specialty.
