Click here for a printable version of this To Do List. 1. Choose the lifestyle disease that your family friend has been diagnosed with. If you’re having trouble coming up with one, click here for some ideas.
2. Confirm your disease choice with your teacher and watch this video. Remember to keep track of your sources throughout this whole process— you’ll need to include a complete list with your final portfolio.
3. Research the disease to discover basic information like the description/definition, causes, symptoms, other names, important vocabulary words, statistics, etc. Use the following websites: -Kids Health -Centers for Disease Control -National Institutes of Health -MSNBC Health -American Lung Association -National Stroke Association -American Cancer Society -National Cancer Institute -American Liver Foundation -American Diabetes Foundation -National Osteoporosis Foundation -American Heart Association Check out these videos, too: -Discovery Health Videos (click Diseases/Conditions tab)
4. Compile and organize this information by creating a graphic organizer using Inspiration. This is an example of what you might create. When it’s done, print it out in color and keep it safe for your portfolio.
5. Using your knowledge of this disease and any experience from your real life, create a description about why your friend has developed this disease. This should include factors like: -age, gender, family history, exercise & eating habits, stress level, job situation, drug use, etc.
6. Write this description in Microsoft Word. It should be at least 2 paragraphs long and be grammatically correct. Include a creative introduction and concluding sentence that will grab your reader’s attention! Print this in black/white and keep it safe with your graphic organizer.
7. Remember the goal setting steps we used for your project? Use your knowledge of this disease, the description of your friend, and these goal setting steps to set a health goal for your friend that will help their current situation. Check out this page for more details.
8. Use Microsoft Publisher to create a plan for your friend. Make sure it’s organized and easy to follow—this person is totally stressed out! You may follow the staircase idea if you’d like, but you don’t have to. When this section is done, it will definitely be more than 1 page. This is a template that you can use and/or modify to create your plan. Print the entire plan out in color and keep it safe for your portfolio.
9. Now it’s time to put this portfolio together! Click here for an overview of what you’ll submit. Gather your 3 documents in a small 3-ring binder — please use dividers to identify each section. Create a cover page that includes a title, a related image, the due date, and both partners’ names. Include your complete list of sources at the end. Create an electronic portfolio, too. Make a folder in the Public Folder titled with your last names (i.e. JonesSmith) - save all documents there. Check out this page to find out how you’ll be graded.
10. Share your portfolio with your classmates! We’ll plan a day for all the portfolios to be laid out — you’ll be able to see what these diseases are all about and learn different things that you can do to prevent them in yourself.
11. In your own handwriting, write a personal reflection essay about this WebQuest. What information did you learn? What helped you learn it? Why is your plan the best plan for this person? How did you and your partner work together? What was the best part of this WebQuest? What would you change? How can you use this experience and information in your real life? You are not limited to these questions—they are just mean to start you thinking. Your reflection should be at least 1 full page long.
