The School of Social Welfare, University at Albany
The School of Social Welfare, University at Albany, prepares BS, MSW, and Ph.D. students to provide services and manage programs that provide assistance with such difficulties as homelessness, mental disorders, domestic violence, substance abuse, physical illness, and family dysfunction. The MSW and BS programs are accredited by the Council on Social Work Education. The School is considered among the best social work programs in the United States.
AARP

AARP is an advocacy organization for adults over 50 years of age. The Albany office mobilizes senior volunteers to advocate to legislators, develops policy briefs and organizes for community action.

Albany County NYConnects
Albany County NY Connects is where anyone who needs long term care information and services, their family members and helping professionals can get the information they need to make informed decisions about long term care as well as the assistance needed in linking with services.
Albany County Nursing Home
The Department of Residential Health Care Facilities provides excellence in long term care services to the people of Albany County and surrounding areas.
Albany Guardian Society
The Albany Guardian Society focuses on the needs of older citizens, seeking to improve their quality of life. Albany Guardian Society promotes the exchange of information to educate and enlighten those who share an interest in creating a better life for seniors.
Alzheimer’s Association Northeastern New York Chapter
Alzheimer’s Association is a voluntary organization offering family support, support groups, volunteer respite training and registry, caregiver’s resource center, Safe Return, helpline, education and advocacy services for Alzheimer’s caregivers and families.
Atria Assisted Living
Atria offers independent and assisted living housing as well as memory enhancement programs to older adults.
Bethlehem Senior Services
The Senior Services Department provides information and referrals for Bethlehem residents over the age of 60 living independently. Programs and services include: social programs, Bethlehem Senior Projects, Inc., transportation, food programs and meals, volunteer, health programs, outreach programs, financial, legal, and utility assistance, and housing options.
Catholic Charities of Albany and Schenectady Counties
Caregivers Support Services is an agency of Catholic Charities of the Diocese of Albany with one simple mission - to make life better for those who care for friends and relatives in need.
The Center for Excellence in Aging and Community Wellness, University at Albany
The Center for Excellence in Aging Services (CEAS) is a translational research center that develops, tests and implements innovative practices and policies that address the needs of aging persons, their families, and caregivers.
Daughters of Sarah Senior Community
Daughters of Sarah Senior Community includes a 201-bed skilled nursing facility with specialized Memory Enhancement Unit, the Massry Assisted Living, rehabilitation, day services, hospice and respite.
Centro Civico, Amsterdam, NY
Centro Civico is a grassroots not-for-profit agency providing bilingual, bi-cultural services to the Hispanic community in a rural county. It provides outreach, case management, advocacy, cultural activities, and community development, and builds coalitions to serve the Hispanic community.
Ellis Medicine, Ellis Mental Health
Ellis Medicine is a large healthcare system that includes acute medical care, rehabilitation services, and mental health programs.
Good Samaritan Lutheran Health Care
Good Samaritan Lutheran Health Care Center is a 120-bed skilled nursing facility with rehabilitation services and affiliated assisted living center.
Greene County Mental Health Center
Greene County Mental Health Center is the New York State Office of Mental Health licensed outpatient community mental health clinic treatment service of Greene County.
Homedical Associates
Homedical Associates is a primary medical care practice that serves homebound individuals in the Capital District.
The Independent Living Center of the Hudson Valley
The Independent Living Center of the Hudson Valley seeks to help persons with disabilities do for themselves instead of depending on others to do for them. They also have a 'Money Follows the Person' program focused on helping nursing home residents return to a more independent level of housing.
Jewish Family Services of Northeastern New York (JFS)
JFS is a not-for-profit agency providing information and referral, counseling for adults of all ages, outreach programs for older persons, family life education, homemaker referral services, and an immigrant resettlement program.
Montgomery County Office for Aging, Inc.
The Montgomery County Office of Aging and Adult Services plans, coordinates and monitors services for Montgomery county residents age 60 and older, including informational and referral services, care management, finance assistance, health and home care, insurance and legal assistance, senior housing and social services.
New York State Office for the Aging (SOFA)
NYSOFA is the central State agency to plan and coordinate programs and services for the aging at all levels in both the public and private sectors. It administers various Titles under the Federal Older Americans Act of 1965 as amended, and a variety of State-funded programs that serve the elderly.
New York State Office of Alcohol and Substance Abuse Services (OASAS)
OASAS is the central State agency to oversee addiction services. The agency is working to improve assessment and identification of substance abuse among seniors and improve treatment options.
Northeast Health (NEH)
NEH is a health system comprised of three health organizations (Samaritan Hospital, Albany Memorial Hospital, and the Eddy) representing a continuum of services for older persons including home health, residential retirement, assisted living and dementia facilities, adult day care, inpatient and outpatient geriatric rehabilitation, and outpatient and inpatient health care.
Palliative Home Care Program of Community Hospice
Palliative Care Program provides by a unified team of trained health care professionals that is focused on relief of suffering and quality of life. Palliative care is for seriously ill patients, regardless of age, who are living with an incurable disease and who do not meet hospice criteria.
Schenectady ARC
Schenectady ARC is committed to helping people with developmental disabilities in the Capital Region enjoy healthy, rewarding lives as members of their community.
Senior Services of Albany
Senior Services of Albany provides community-based services to assist older adults and families cope with the issues that may arise with aging.
St. Mary’s Hospital
St. Mary's Hospital is a 143 bed acute care hospital located in Amsterdam. The hospital also provides primary and family care programs, complementary therapies, health and wellness education and support programs.
St. Peter's Health Care Services
St. Peter's Health Care Services is a large health service network, located in Albany, that includes an acute care hospital, rehabilitation program, nursing home, home care program, specialized aftercare management, hospice, and wellness programs.
Stratton Veterans Administration Medical Center, Department of Veterans’ Affairs (VAMC)
VAMC is a government health system for military veterans representing a unique model of managed care with community health clinics and primary care sites throughout northeastern New York. The VAMC provides home care, nursing home, adult day care, respite, specialized dementia care, a homeless program, adult foster care, and the full range of inpatient and outpatient medical and allied health service.
The Cardiac Wellness Center of Capital Cardiology
The Cardiac Wellness Center offers physician-directed exercise, nutrition and cardiac disease risk-management programs for all ages. Programs are adapted from cardiac rehabilitation programs developed to help patients with coronary artery disease resume a normal lifestyle and reduce risk of disease progression following a heart attack, angina or heart surgery.
Whitney M. Young, Jr., Health Center (WMY):
WMY is a not-for-profit urban ambulatory care center providing medical and mental health care for children and adults as well as other programs including methadone maintenance, Women, Infants, and Children (WIC), dental, family alcoholism, and an HIV treatment and prevention program. Located in downtown Albany, this agency serves an urban, low-income, racially diverse population.

John A. Hartford Foundation
Hartford Foundation is the original funder of the Geriatric Social Work Practicum Program, of which the IAP is a member. The Hartford Foundation supports efforts to improve health care in America.
New York Academy of Medicine
The New York Academy of Medicine coordinates the Geriatric Social Work Practicum Program. The Academy is an independent academic institution committed to improving the health of urban populations, particularly the urban poor, and to the continuing development and scholarship of health care professionals. The New York Academy of Medicine is also home to the Social Work Leadership Institute.
Council on Social Work Education (CSWE) SAGE-SW
With funding from the John A. Hartford Foundation, the Council on Social Work Education (CSWE) has launched Strengthening Aging and Gerontology Education for Social Work (SAGE-SW). SAGE-SW is dedicated to preparing ALL social workers to meet the needs of a growing aging population through faculty development institutes, conferences and training grants.
