How to Apply

Please review the program's prerequisites to be sure you are eligible to apply.
Download a copy of our application form, and submit the following materials to the address below by November 1st, 2007. Letters and transcripts must be mailed directly from the source, not through the applicant. Since we do not participate in the National Residency Matching Program (NRMP) all applications come directly to us.

	Completed application form
	Personal Statement (2 pages max.) describing your interest in preventive medicine and your career goals
	Curriculum vitae
	3 original Letters of Recommendation (Letters must be sent directly from the source and not through the applicant. One of these letters must be from your program director if you are a current or recent resident or fellow)
	Previous Residency Diploma or Letter sent directly from your Residency Program confirming dates attended
	Medical School Transcript - sent directly from the school
	Grad School Transcript (if applicable) - sent directly from the school
	College Transcript - sent directly from the school
	Proof of NYS Medical License Eligibility: copy of USMLE Part III or FLEX, proof of at least ONE PGYear of training (for US graduates) or THREE PGYears (for Foreign graduates)
	Photo (optional)
