How to Apply

Please review the program's prerequisites to be sure you are eligible to apply.

Starting September 1, 2009, our program will be accepting applications through Electronic Residency Application Service (ERAS). All applications for July 1, 2010 admissions should be submitted through ERAS.

In your application, among other documents required by ERAS, please include:
Personal Statement (2 pages max.) describing your interest in preventive medicine and your career goals
Curriculum vitae
At least 3 Letters of Recommendation; one of these letters must be from your program director if you are a current or recent resident or fellow. If you are currently employed, you will need to submit a letter from your supervisor.
Previous Residency Diploma or Letter from your Residency Program confirming dates attended
Medical School Transcript
Grad School Transcript (if applicable)
College Transcript
Proof of NYS Medical License Eligibility: copy of USMLE Part III or FLEX, proof of at least ONE PGYear of training (for US graduates) or THREE PGYears (for Foreign graduates). If accepted, NYS license should be obtained prior to the start of the program (July 1). For more information on medical license, please visit NYS Office of the Professions
