Pay For Performance: Aligning Incentives for Improved Quality and More Efficient Care

The Institute of Medicine’s 1999 report on Quality of Healthcare in the U.S. and the more recent Medicare Modernization Act of 2003 have led to the development of pay for performance (P4P) models, shifting healthcare provider incentives toward more efficient healthcare, improved quality of care, and greater patient centeredness. The Institute of Medicine has also just released a report dedicated to an analysis of P4P, calling it a potentially major development in health care quality improvement. In the Capital Region, most know of the Bridges to Excellence (BTE) program, driven by large, local employers. There are numerous P4P demonstrations ongoing throughout the nation. This session focuses on the concept of P4P and how to think about benchmarks and measures for implementing P4P programs. It reviews the national picture and evaluation of P4P models to date, and raises key issues that all P4P programs must address. Through the lens of various stakeholders - employers, payers, and providers – an assessment of Capital Region best practices in P4P will be presented, in addition to discussing what the future might hold for this novel concept.
