Healthy Children New York
Save the Date - Regional Trainings
The goal of Healthy Children New York is to promote the health of
children and prevent disease, to support optimal health for children.
This goal will be met through a statewide network of Child Health
Promotion Specialists who will act as a public health resource to the
child-serving community. Their specialized training as a CHPSs address
the issues raised in the preceding needs statement, as well as many
others. The School of Public Health is working with the Bureau of Child
and Adolescent Health, NYS Department of Health, to provide training
for this initiative.

Monthly Log of Program Development and
Other In-office Activities
This monthly log is used to record telephone and email consultations, faxes, mailings of items to individual agencies and time spent on program development activities. It is NOT to be used for site visits, trainings, mass mailings, meetings with other agencies, or other encounters that typically last an hour or more.

Please submit one completed form for each of the following types of encounter you have with an external agency:
Site Visit, Training, Mass Mailing, Meeting with other agency, Other encounters typically lasting an hour or more.

they are NOT to be reported using this form.
