In his plenary address at the Institute for Healthcare Improvement's (IHI) 18th Annual National Forum in December 2006, Donald M. Berwick, MD, President and CEO of the Institute for Healthcare Improvement announced the IHI's new initiative, the 5 Million Lives Campaign. Dr. Berwick reflected on the achievements of hospitals participating in the IHI 100,000 Lives Campaign and announced the next campaign step, saying, "We're going after harm." With an estimated 15 million injuries in hospitals each year the goal was set to prevent five million "incidents of medical harm" over a two year period.
