What forms do I need for my internship? 

All internships will be logged electronically through Handshake. Students are required to register their internship and receive digital approval from their supervisor, track hours while working at their internship, and upon completion of the current internship term, upload a signed copy of their hour logs to their experience, evaluate their placement site and be evaluated by their supervisor. This process MUST be repeated for every internship a student holds. If a student is continuing the same internship placement into a new semester (ex. Continuing a summer internship into the fall semester) then they MUST resubmit all forms when the fall semester begins. The only document we would not request again is the student and supervisor expectation form.

Failure to submit the forms required for an internship in a timely manner may result in that internship not counting towards graduation requirements.

If a student has met the minimum requirement of internship hours for their specific graduate program, but are continuing to complete internships, the career office requests that the student continue to document their experience in order for the office to accurately track where our students are gaining experience and which employers are continuing to provide these opportunities. The only document the office would request for these students at the conclusion of the experience is their hour log.

When you have received an offer:

Create a “New Experience” on Handshake. This will notify the Office of Career Development of your hire. We will send a request for your supervisor’s digital approval of your placement. During that time, the career staff will upload your hour log and student and supervisor expectation form. The student and supervisor expectation form is due no later than two weeks after starting your placement. This document must be uploaded as a new attachment to their experience on Handshake.

While employed at your internship:

Make sure to keep track of your hours worked. You will need to submit your total number of hours worked at the end of each term. Please note that these logs provided by the office are the main document we utilized for tracking of hours. Time sheets from your placement will not be counted as documentation.

Upon the end of the internship term:

Students must evaluate their internship site upon completion of a given term. Students are also responsible for making sure their supervisor submits an evaluation of them through Handshake.. Evaluations are sent to the student and supervisor as a digital request from Handshake. Lastly, the student and supervisor must sign and date the completed hour log and the student must upload and attach it to their experience on Handshake.
