NOTE: If your mentor has already filled out page seven of the Intel STS form, you may use that in lieu of the JSHS "Scientist Certification" form. Likewise, if your research teacher has filled out page eight of the Intel STS form, it may be used in place of the JSHS "Teacher Certification" form.
