Need A Passport?

Passport Application Day will be held on Tuesday, December 4th from 10:00 a.m. to 3:00 p.m. in the Mail Services facility (basement level, corner of Business Administration & Social Science buildings). Call (518) 442-3271 to arrange an appointment or obtain additional information. (Walk-ins are welcome if time allows.)

You'll need to provide the following items when you submit your passport application in person:

Passport Application

• Passport application form DS-11 (unsigned) - available from Mail Services or www.usps.com/passport

Proof of U.S. citizenship (provide one of the following)

• Previous U.S. passport
• Certified U.S. birth certificate (must have raised seal)
• Consular report of birth abroad or certification of birth
• Certificate of naturlization or citizenship

Proof of identity (provide one of the following):

• Certificate of naturalization or citizenship
• Current, valid driver's license
• Government ID
• Military ID

2 identical passport photos (available at event for $15):

• 2 x 2 inches in size
• Taken within the past 6 months
• Color preferred
• Full-face, front view with white background
• Normal street attire; no hats or headgear

Passport fees:

• U.S. Department of State total: $67
• USPA Acceptance Fee: $30
• USPS fee for photos at location: $15

U.S. Department of State total may be paid with check or money order. USPS acceptance fee (and photo fee) may be paid with cash, check, money order, or credit/debit card.

Your passport will take approximately 8 weeks to complete the process and be returned to you.
