Application Payment Form Instructions

All applicants must submit the required, nonrefundable application fee before the application can be processed and an application number can be provided.

Each fee must be submitted as a check or money order made payable in U.S. dollars, or by completing the credit card payment authorization on the Application Fee Payment Form. The complete name of the applicant must be written on the front of the check or money order.
