Things to do

The important things to do before departure are:

l.) STATEMENT OF RESPONSIBILITY,RELEASE AND ASSUMPTION OF RISK form, signed and dated.

2.) The following information:
-Passport Number (or Citizenship + visa)
-Date of birth
-Social Security Number
-Emergency Contact
-Emergency Phone Number

3.) Please send me (or drop off in my office: Humanities 219) the Partners in Progress Waiver of Responsibility Form (I have copies in the office, if you cannot download.)

4.) Save the afternoon of January 31st for the first of several ORIENTATION TO HAITI seminars.

5.) Take your immunizations.

6.) Check your health insurance.
