Forms can be turned in at CC137 in the Campus Center:

- between 8:30 AM and 5:00 PM during the semester
- between 8:00 AM and 5:30 PM during finals

Accomodation Request (PDF) (MS Word)
Test Schedule (PDF) (MS Word)

Students may complete the form as a WORD doc and send it to the office as an Word doc attachment via email OR they may scan the printed completed form (Accommodation Request or Testing Schedule) and email it to us as a PDF attachment to AArminio@uamail.albany.edu .
