Independent Study Form: Get Form pdf

What is it used for?	To register for an independent project working with a School of Business faculty member. The student must also submit a one-page description of the project and the deliverable.
Who needs to sign it?	Instructor Note: Department approval is NOT needed.
Where does it go?	After the form has been filled out and signed, the student should bring the form to the Office of Student Services.

Transfer of Credit Form: Get Form word / pdf

What is it used for?	To transfer graduate course credit from another institution towards a student's University at Albany degree.
Who needs to sign it?	Albina Grignon at the Office of Student Services
Where does it go?	To the School of Business Student Services Office. An official transcript also needs to be sent to the Office of Student Services.
