Please consult with the Assistant to the Chair if you have specific questions. For security, these forms are located on the Anthropology share drive and can be accessed by faculty and staff only.

Description
Admit to Candidacy
Student must have completed 2 semesters of full time study in residence, foreign language, both questions, and any other requirements specifically called for by GAC.
Advanced standing in PhD program Advisors may petition GAC on the behalf of students admitted to the PhD program to use credits from other institutions to fulfill phase 1 requirements. This is forward from GAC to the Assistant to the Chair and then to the Registrar. A maximum of 30 credits can be applied to Phase 1 of PhD program.
Confer PhD Use this form after the student has completed all of his/her requirements and the dissertation has been approved.
Data form Students should update this document every semester or as they pass benchmarks. Assiduously updating this form will reduce stress at the end of Phase 2.
Dissertation Proposal defense announcement This is simply a memo from the advisor indicating the time, date and title of the proposal defense.
Form Dissertation Committee Formal declaration of the committee that needs to be submitted and approved by the GAC.
Report Comp exam score This form should be filled out and handed in to the Assistant to the Chair for forwarding.
Report Completion of Both Questions This form needs should be filled out and handed in to the Assistant to the Chair for forwarding. Should be completed after the second question is approved.
Report Completion of Foreign Language requirement Students should provide the Assistant to the Chair with proof of completion of the foreign language exam or course from the designated examiner.
Report Completion of Research Requirement (Masters paper) This memo should be completed upon completion of the masters paper. A copy of the masters paper should be kept in the department office in the student's academic file.
Transfer credit to Masters program This form is for masters students who wish to bring in one or two courses from a previous institution. Give to the Assistant to the Chair who will forward it to Graduate Admissions (J. Champagne).
