The West Hill Neighborhood Health Advocate Program (WHNHAP) is a service project of the Center for Excellence in Aging Services where it continues to enrich the West Hill community exemplifying the work, mission and goals of the Center. Originally developed by Robert Paeglow, M.D. of Koinonia Primary Healthcare in collaboration with Albany Guardian Society and the Community Foundation of the Capital Region, WHNHAP was established as a faith-based initiative in the West Hill neighborhood of Albany, New York in coordination with recommendations of a planning grant for the John A. Hartford Aging Prepared Community Initiative.
Today, the project is funded in part by Healthy Hearts on the Hill, a new collaborative initiative of the Center’s with Koinonia Primary Healthcare, a community wide initiative aimed at increasing awareness and participation in heart healthy behaviors. Enlisting the support of a community coalition, WHNHAP lay advocates and community partners develop capacity, recruit participants and deliver services that will embed evidence-based health promotion programming and practices in disease self-management, physical activity and nutrition into the fabric of the West Hill community. Healthy Hearts on the Hill is supported by the Atlantic Philanthropies and the John A Hartford Foundation.
The lay advocates are supported by Koinonia Primary Healthcare, a team of health care professionals including a social worker, social work interns, registered nurses, nursing interns, and medical students, a dedicated group of faith-based communities and the Healthy Hearts on the Hill Community Coalition.
The West Hill Neighborhood Health Advocate Program facilitates the collaboration and coordination of many informal help networks in the West Hill community. This project does not replace existing services, but enhances the utilization of such services by West Hill seniors and their caregivers.
The West Hill Neighborhood Health Advocate Program’s office is located in the heart of the West Hill community at 553 Clinton Avenue. This space has been graciously donated through the efforts of Robert Paeglow, M.D. of Koinonia Primary Healthcare and Pastor Stephen Lalor of the Capital District Prayer and Healing Center.
Program Goals
- To reduce or delay the institutionalization and hospitalization of the elderly
- To link health and social services providers and faith communities to enhance the provision of care to the elderly with unmet needs
- To provide opportunities for healthy seniors to contribute to their community
- To promote a multigenerational model of care
- To share useful information to help people deal with the complexities of today’s health care environment
Where is West Hill?
The West Hill community is located on the west side of the city of Albany. It is located between Henry Johnson Boulevard, North Manning Boulevard, Central Avenue and Livingston Avenue.
